Finasteride (Propecia) for Hair Loss in Hong Kong: What You Need to Know

No items found.

Finasteride — sold under the brand name Propecia® — is the most widely prescribed oral medication for male pattern hair loss worldwide. If your doctor has mentioned finasteride or you've been researching treatment options, here's what the evidence says and what to expect as a patient in Hong Kong.

For a complete overview of all AGA treatments, see our Complete Guide to Hair Loss Treatment in Hong Kong.

What Is Finasteride?

Finasteride is a selective Type II 5-alpha reductase inhibitor. In simpler terms, it blocks the enzyme that converts testosterone into dihydrotestosterone (DHT) — the androgen responsible for shrinking hair follicles in men with androgenetic alopecia (AGA).

By reducing serum DHT by approximately 70%, finasteride slows or halts follicular miniaturisation and, in many men, allows partially miniaturised follicles to recover.

The drug was originally developed at a higher dose (5 mg) for benign prostatic hyperplasia (BPH). In 1997, a 1 mg dose was approved specifically for AGA after clinical trials demonstrated significant efficacy.

The Evidence

The Landmark Trial

Olsen et al. conducted one of the pivotal trials for finasteride 1 mg in AGA. Over 48 weeks, men receiving finasteride showed statistically significant increases in hair count compared to placebo, with benefits continuing through the second year of treatment (Kaufman KD, Olsen EA, et al. J Am Acad Dermatol. 1998;39:578-589. PMID: 9777765).

Key findings from this and subsequent studies:

  • 83% of men maintained or increased hair count over 2 years(Based on EAU Guidelines; prescribing information)
  • 66% of men showed visible improvement in hair growth(Based on EAU Guidelines; prescribing information)
  • Benefits were seen at both the vertex and anterior mid-scalp
  • Improvements continued up to 24 months, then generally stabilised

How It Compares

A recent network meta-analysis placed finasteride 1 mg among the most effective monotherapies for AGA, though dutasteride 0.5 mg showed potentially greater efficacy in hair count improvement (Gupta AK, Bamimore MA. J Cosmet Dermatol. 2024;23:2964-2972. PMID: 38725143).

The Japanese Dermatological Association gives finasteride a Grade A recommendation — the highest evidence rating — for treating male AGA (日本皮膚科学会 男性型脱毛症診療ガイドライン 2017).

How to Take Finasteride

  • Dose: 1 mg once daily
  • Timing: Same time each day, with or without food
  • Duration: Indefinite — AGA treatment is ongoing. Stopping leads to resumed hair loss within 6–12 months(Based on EAU Guidelines; prescribing information)
  • Timeline for results:
  • 1–3 months: Reduced shedding (some experience temporary increased shedding)
  • 3–6 months: Early signs of improvement
  • 6–12 months: Visible results
  • 12–24 months: Full effect

Side Effects

The most discussed concern with finasteride is sexual side effects. Here's what the clinical data shows:

From the Olsen et al. trial (PMID: 9777765):

  • Decreased libido: Reported by approximately 1.8% (vs 1.3% placebo)
  • Erectile dysfunction: Approximately 1.3% (vs 0.7% placebo)
  • Reduced ejaculate volume: Approximately 0.8% (vs 0.4% placebo)

These rates are low and only marginally higher than placebo. Most side effects resolved with continued treatment or upon discontinuation.

Other Potential Side Effects

  • Breast tenderness or gynecomastia — rare (<1%)(Based on EAU Guidelines; prescribing information)
  • Mood changes — reported anecdotally; not consistently demonstrated in controlled trials(Based on EAU Guidelines; prescribing information)
  • Allergic reactions — very rare

Post-Finasteride Syndrome

There are online reports of persistent sexual or neurological symptoms after stopping finasteride ("post-finasteride syndrome"). This remains controversial: controlled studies have not established a clear causal link, but some patients report real symptoms(Based on EAU Guidelines; prescribing information). If you have concerns, discuss them with your prescribing doctor.

Important Safety Information

  • Women — Finasteride is contraindicated in women, especially those who are or may become pregnant. The drug can cause genital abnormalities in male foetuses. Women should not handle crushed or broken tablets.
  • Blood donation — Wait at least 1 month after stopping finasteride before donating blood(Based on EAU Guidelines; prescribing information).
  • PSA testing — Finasteride can lower PSA levels by approximately 50%. Inform your doctor if you're having prostate screening.

Maximising Your Results

Combine with Minoxidil

For many men, finasteride alone is enough to halt hair loss and achieve some regrowth. But for those wanting maximum results — especially with moderate to advanced thinning — adding topical or oral minoxidil provides complementary benefits.

Finasteride blocks the cause (DHT), while minoxidil stimulates growth through a separate mechanism (Messenger AG, Rundegren J. Br J Dermatol. 2004;150:186-194. PMID: 14996087). Together, they address AGA from two angles.

Start Early

The single most important factor in AGA treatment outcomes is timing. Finasteride is far more effective at maintaining existing hair than regrowing hair from long-dormant follicles. Starting at the first signs of thinning — even if it feels "minor" — gives you the best shot at long-term results.

Be Consistent

Take your tablet at the same time every day. Missing occasional doses is unlikely to derail treatment, but consistency maximises steady-state DHT suppression.

Be Patient

Don't judge results at 3 months. Give it at least 12 months before making any decisions about efficacy. Some men experience increased shedding in the first few months — this is actually a positive sign that the hair cycle is resetting.

Frequently Asked Questions

1. Is finasteride safe long-term?

Clinical trial data extends to 5+ years, showing maintained efficacy and a consistent safety profile(Based on EAU Guidelines; prescribing information). Many men take finasteride for decades. Long-term safety monitoring has not revealed new concerns beyond those identified in original trials(Based on EAU Guidelines; prescribing information).

2. Can I take a lower dose to reduce side effects?

Some doctors prescribe finasteride at lower doses (e.g., 0.5 mg or even 0.25 mg daily). Research suggests that even 0.2 mg/day achieves meaningful DHT suppression, though slightly less than 1 mg(Based on EAU Guidelines; prescribing information). Discuss dosage adjustments with your doctor.

3. What about topical finasteride?

Topical finasteride formulations are emerging as an alternative that may reduce systemic exposure while maintaining scalp DHT suppression(Based on EAU Guidelines; prescribing information). These are not yet widely available in Hong Kong but may become an option in the future.

4. Will finasteride work on my hairline?

Finasteride has shown efficacy at both the vertex (crown) and the anterior mid-scalp region. However, frontal hairline recession is generally more resistant to treatment than crown thinning(Based on EAU Guidelines; prescribing information). Setting realistic expectations is important.

5. What if finasteride doesn't work for me?

If finasteride hasn't produced satisfactory results after 12+ months of consistent use, your doctor may recommend switching to dutasteride (which achieves greater DHT suppression) or adding minoxidil to your regimen.
