Minoxidil is the only hair loss treatment available without a prescription in Hong Kong — and it's been one of the most widely used treatments for androgenetic alopecia (AGA) for over three decades. Whether you're considering topical or oral minoxidil, here's what the evidence shows.
→ For a complete overview of all AGA treatments, see our Complete Guide to Hair Loss Treatment in Hong Kong.
What Is Minoxidil?
Minoxidil was originally developed as an oral antihypertensive medication in the 1970s. Doctors noticed an unexpected side effect: hair growth. This led to the development of topical formulations specifically for hair loss.
Unlike finasteride and dutasteride, minoxidil does not block DHT. It works through an entirely different mechanism — which is precisely why it's so valuable as a complementary treatment.
How Minoxidil Works
Messenger and Rundegren explained minoxidil's mechanisms in their landmark review:
- Vasodilation: Minoxidil and its active metabolite (minoxidil sulphate) open potassium channels in vascular smooth muscle, increasing blood flow to hair follicles
- Anagen prolongation: It extends the growth (anagen) phase of the hair cycle
- Follicle enlargement: Miniaturised follicles increase in size, producing thicker hair shafts
(Messenger AG, Rundegren J. Br J Dermatol. 2004;150:186-194. PMID: 14996087)
The exact cellular mechanisms are still being researched, but the clinical effect is well-established: minoxidil promotes hair growth independently of the androgen pathway.
Topical vs Oral Minoxidil
Topical Minoxidil
This is the classic formulation — available as a liquid solution or foam, typically at 2% or 5% concentration.
How to apply:
- 1 mL of solution (or half a capful of foam) to affected areas of the scalp
- Twice daily (5% solution) or once daily (5% foam) — follow product instructions
- Apply to dry scalp
- Leave on for at least 4 hours before washing
- Wash hands thoroughly after application
Pros:
- Available over-the-counter in Hong Kong (no prescription needed for topical)
- Well-established safety profile
- Localised treatment — minimal systemic absorption
Cons:
- Requires consistent daily application — easy to skip
- Can leave scalp greasy or flaky (solution formulation)
- May cause scalp irritation, especially the alcohol-based solution
- Takes time to apply properly
Oral Minoxidil (Low-Dose)
In recent years, low-dose oral minoxidil (typically 2.5–5 mg daily) has gained traction as an off-label AGA treatment. It eliminates the hassle of topical application and may provide more uniform scalp coverage.
Gupta & Talukder provided a comprehensive comparison of oral minoxidil alongside finasteride and dutasteride for AGA (Gupta AK, Talukder M. J Dermatolog Treat. 2022;33:2946-2962. PMID: 35920739).
Pros:
- Convenient — one tablet daily
- No topical application hassle
- Potentially more effective due to systemic delivery(Based on EAU Guidelines; prescribing information)
Cons:
- Prescription required in Hong Kong
- Systemic side effects possible (see below)
- Off-label use — originally an antihypertensive
- Requires medical monitoring, especially initially
Efficacy
What Does the Research Show?
Topical minoxidil 5% is more effective than 2% for AGA(Based on EAU Guidelines; prescribing information). The Japanese Dermatological Association's 2017 guidelines give topical minoxidil a Grade A recommendation for male AGA — the highest evidence level (日本皮膚科学会 男性型脱毛症診療ガイドライン 2017).
Key findings from the evidence base:
- Vertex (crown) responds better than the frontal hairline(Based on EAU Guidelines; prescribing information)
- Peak effect at 12–16 months of consistent use(Based on EAU Guidelines; prescribing information)
- Combination with a DHT blocker is more effective than minoxidil alone (Gupta AK, Bamimore MA. J Cosmet Dermatol. 2024;23:2964-2972. PMID: 38725143)
The Shedding Phase
Many new minoxidil users experience increased shedding in the first 2–8 weeks. This is normal and actually a positive sign — it indicates that the hair cycle is being reset, with resting (telogen) hairs being pushed out to make way for new anagen hairs.
Don't stop treatment because of early shedding. It's temporary.
Side Effects
Topical Minoxidil
- Scalp irritation — the most common complaint, particularly with alcohol-based solutions. Foam formulations are generally better tolerated.
- Contact dermatitis — rare allergic reaction(Based on EAU Guidelines; prescribing information)
- Facial hypertrichosis — unwanted facial hair growth, more common in women but can occur if the product drips onto the face
- Initial shedding — temporary, as discussed above
Oral Minoxidil
Because oral minoxidil is absorbed systemically, it carries additional considerations:
- Hypertrichosis — increased body and facial hair is the most common side effect, reported in a significant proportion of users(Based on EAU Guidelines; prescribing information)
- Fluid retention/oedema — particularly at higher doses(Based on EAU Guidelines; prescribing information)
- Dizziness/lightheadedness — due to blood pressure lowering(Based on EAU Guidelines; prescribing information)
- Palpitations/tachycardia — rare at low doses, but cardiovascular monitoring is recommended(Based on EAU Guidelines; prescribing information)
- Pericardial effusion — extremely rare at AGA doses, but historically reported at higher antihypertensive doses(Based on EAU Guidelines; prescribing information)
Important: Oral minoxidil should be prescribed and monitored by a doctor. It is not appropriate for self-medication.

Maximising Minoxidil Results
1. Combine with a DHT Blocker
Minoxidil treats the symptom (poor growth), while finasteride or dutasteride treats the cause (DHT). Using both gives the best outcomes for most men.
→ Read more: Finasteride vs Dutasteride: Which Is Better?
2. Be Consistent
For topical: apply every single day, ideally at the same times. Inconsistency is the #1 reason for poor results.
For oral: take as prescribed, same time daily.
3. Apply Correctly
If using topical, ensure the scalp is dry before application. Part your hair to expose the thinning areas. Use a dropper or the foam applicator to apply directly to the scalp — not to the hair.
4. Give It Time
Minimum 6 months before assessing results. Full effect at 12+ months. Early shedding is normal and temporary.
5. Don't Stop Suddenly
Like all AGA treatments, hair growth benefits are maintained only while you continue treatment. Stopping minoxidil leads to gradual loss of the gains, typically within 3–6 months(Based on EAU Guidelines; prescribing information).
Frequently Asked Questions
1. Can I use minoxidil without a prescription in Hong Kong?
Yes — topical minoxidil (up to 5%) is available over-the-counter at pharmacies. Oral minoxidil requires a prescription from a registered doctor.
2. Which is better: minoxidil or finasteride?
They work through different mechanisms and are most effective when used together. If you have to choose one, finasteride addresses the underlying cause (DHT) while minoxidil stimulates growth. Your doctor can help decide based on your situation.
3. Can women use minoxidil?
Topical minoxidil 2% is approved for female pattern hair loss(Based on EAU Guidelines; prescribing information). The 5% formulation and oral minoxidil are also used off-label for women. However, this article focuses on male AGA — women should consult a doctor for personalised advice.
4. Does minoxidil work on a receding hairline?
Minoxidil is most effective on the vertex (crown). Results on the frontal hairline and temples are more variable and generally less impressive(Based on EAU Guidelines; prescribing information). For hairline-specific concerns, a DHT blocker may be more effective.
5. What happens during the initial shedding phase?
Increased shedding in weeks 2–8 is normal. Minoxidil pushes resting hairs out of the follicle to make room for new growing hairs. This is temporary and usually resolves within 2–3 months. Don't stop treatment.
